Claims process

In relation to Life and Disability claims, please note that:

  1. Unless otherwise specified, it is the Policyholder (Company's) responsibility to notify us and submit any supporting documents within:
    • two years of the death of a group member or within 24 months of the end of the waiting period or, in the absence of a waiting period within 24 months of the event that first led to the claim.
    • The claim must be notified to us within the notification period set out above. Any failure to notify the claim within the prescribed notification period will lead to rejection of the claim.
  2. Unless otherwise specified, Zurich Eurolife has no obligations in respect of events occurring after the end date of the insurance cover.
  3. The Policyholder (Company) must retain any original supporting documents (e.g. receipts) when sending us copies, as we reserve the right to request the original documents/receipts for fraud detection purposes up to 12 months after settling claims. In addition, we advise the company to keep copies of all correspondence with us, as we cannot be held responsible for correspondence that does not reach us for any reason that is outside of our control.
  4. Eligible benefits will be paid in the currency specified in the policy schedule.
  5. The Policyholder (Company) agrees to help us to obtain all necessary information to process a claim. We have the right to access all medical records and to have direct discussions with the medical provider or the doctor treating your employee. We may, at our own expense, request a medical examination by our medical representative when we consider this necessary. All information will be treated in strict confidence.
  6. We reserve the right to restrict the benefit payment to some countries which are subject of trade or economic sanctions laws and regulations.
  7. Unless otherwise specified, the policyholder is responsible for paying all benefits and sums owed to eligible beneficiaries and insured persons under this policy. The policyholder is also responsible for filing any income tax returns associated with these benefits. We are not responsible for withholding any taxes from such payments, nor for filing any income tax returns associated with these benefits. We do not accept any responsibility for changes in taxation or legislation that occur after the policy start date.

To claim benefits, the policyholder must send us the following documents, as explained.

Requirements for Death claim (GL, SI, CI, AD)

  • Death Claim form to be duly signed by authorized signatories of the policyholder as per Trade Register/signatory power/signatory list
  • A valid & certified* proof of ID of each authorized signatory
  • Original Death Certificate or a certified true copy of the original Death Certificate
  • Salary slip of the insured at the claim event date
  • Proof of Employment at the claim event date
  • A Police or an Accident report (only applicable if death is due to an accident or unnatural circumstances)
  • Autopsy report or coroner’s inquest, if available

Requirements for a disability claim (ADIS, IP, TTD, PPTD, PTD)

  • Disability Claim form to be duly signed by authorized signatories of the policyholder as per Trade Register/signatory power/signatory list
  • Disability Claim Assessment form to be duly signed by the Insured and by the medical practitioner and with supporting reports available
  • Sick Notes or Confirmation of absence periods
  • Salary slip of the insured at the claim event date
  • Proof of Employment at the claim event date
  • A Police or an Accident report (only applicable if disability is due to an accident or unnatural circumstances)
  • A valid & certified* proof of ID of each authorized signatory

* We will only accept certified documents when the certification is carried out by one of the following: broker, company’s secretary, police officer, court clerk, notary public, practicing solicitor, embassy, consular staff member, commissioner for oaths.

In the event of the death, accidental death or accidental dismemberment of an insured person, the policyholder must notify us of the event in writing. The policyholder must send us all required documents as indicated above.

Benefits will be paid once we accept the claim as valid. Valid benefits in relation to Life and Accidental Death cover will be paid directly to the Policyholder(Company).

The policyholder must inform us in writing immediately if an insured employee is on certified sick leave

The policyholder must send us the documents as per requirements stated above. We will pay the benefits once we accept the claim as valid.

The policyholder must tell us in writing immediately if an insured employee progresses from short-term disability to long-term disability. The policyholder must send us the documents as per requirements stated above.

We will pay the benefits once we accept the claim as valid.

The policyholder must tell us in writing within three months after waiting period if an insured employee suffers a permanent disability. The policyholder must send us the documents as per requirements stated above. The assessment of claims will normally take place after waiting period, based on their individual circumstances.

We will pay the benefits once we accept the claim as valid.

The claim assessment is subject to the receipt of the requirements stated above. The claim handlers will review each provided file and we reserve the right to request further information if we see fit. Once the file is complete and the claim is in scope of the benefit definition as per the Policy schedule document, we would consider the claim valid and accepted.

The following reasons can lead to a declined decision though:

  • late notification
  • min. degree of disability not reached
  • waiting period not reached
  • member has returned back to work
  • premium for the period when the claim occurred was not paid
  • the claimant did not meet the actively at work requirements when entering the plan
  • policy has been terminated
  • exclusions applicable
  • member not eligible / not insured
  • failure to supply the information necessary to proceed with the decision
